$750 Sign on Bonus Are you passionate about assisting people who need help in obtaining employment and independence? Northeast Career Planning and Unity House of Troy is looking for the right person to join the team. This is a full-time, Monday-Friday 37.5 hours /week. Hours are typically 8-4 with occasional adjustments to accommodate the participant. Responsibilities: Serve as the first point of contact for new referrals to Employment Services. Conduct intake interviews to gather background information, assess needs, and determine service eligibility. Present cases to the Prep Committee for review and service planning. Maintain accurate and timely documentation in electronic health records. Ensure intake procedures align with ACCES-VR and Unity House of Troy standards Administer individualized specialized testing and assessments using tools to measure academic achievement, aptitudes, cognitive abilities, personality, vocational interests, and sensory/motor skills. Analyze data from mock interviews, records, and evaluations to identify employment barriers and service needs. Prepare comprehensive evaluation reports with recommendations, adhering to CRS contract timelines. Collaborate with ACCES-VR Vocational Counselors to advocate for appropriate services. Participate in review sessions with participants and referral sources as needed Maintain a small PRE-ETS caseload to compliment the Vocational Evaluation and testing in a school setting. Build and maintain relationships with referral sources, including schools and community organizations. Track and report program metrics (units, hours, dollars) to support program viability. Submit accurate and timely billing. Stay current on ACCES-VR regulations and contract requirements. Promote an environment of inclusion and belonging. Requirements: Bachelor's degree required, Master's degree in related field strongly preferred. At least 2 years of experience in a front facing customer service role. Prior experience working with people with disability. Valid & clean NYS driver's license and access to reliable transportation required. Strong interpersonal, organizational, and documentation skills. Ability to work independently and collaboratively in a fast-paced environment. Ability to adhere to established ACCES-VR documentation deadlines. Proficiency in electronic health records and data interpretation preferred. Experience with intake procedures and vocational evaluations and a working knowledge of ACCES-VR policies, service expectations and billing methodology for CRS service codes is helpful.
