The Social Worker 2 provides advanced skilled interventions for the support and/or resolution of patient and family crises, problem solving and decision-making advocacy, facilitation necessitated by life-changing events, mediation of risk factors, coping with disease or disability, and other psychosocial and/or socioeconomic issues. Is a resource for the Social Worker 1. Education. Masters Degree in Social Work or related field required. Certification, Registration & Licensure. Experience. Minimum of three years clinical Social Work in a hospital, Skilled Nursing facility, Home Health or Hospice setting. Demonstrates effective interpersonal, organizational and motivational skills.
