You'll:

• Create Individualized Plans based on physical, emotional, and environmental assessments.
• Provide Hands-On Care that prioritizes comfort, symptom management, and quality of life.
• Collaborate with Confidence by communicating vital updates to physicians, supervisors, and your interdisciplinary team.
• Support Families Compassionately, guiding them through the hospice journey with education and empathy.
• Act Proactively, quickly responding to any changes in patient condition and adjusting care plans as needed.
• Deliver Excellence with every visit, upholding the highest standards of clinical care and patient dignity.

What You Bring

• Graduation from an accredited nursing program (Diploma, ASN, or BSN)
• Current RN license in Michigan
• Minimum 1 year of clinical nursing experience
• 1 year of Hospice or Home Health experience is required
• Reliable transportation and a commitment to patient-centered fieldwork
• Ability to multitask, document thoroughly, and adapt with grace
• Physical ability to lift up to 50 lbs and perform intermittent physical activity
