Primary Function/General Purpose of Position

The Human Services Assistant assists the case management team with the delivery of clinical case management services to assigned patient populations. Services include but are not limited to outreach, monitoring, service planning, financial resource planning, case coordination, advocacy, education, and follow-up directly related to the patient.

Job Summary Specific to HIV Grant Program

The Human Services Assistant assists the medical case management team with the delivery of clinical case management services to persons living with HIV and their loved ones through the provision of a comprehensive system of case management. Services include insurance and benefits navigation, monitoring, service planning, financial resource planning, crisis intervention and prevention, case coordination, advocacy, education, and follow-up directly related to the patient. The individual must be nonjudgmental, culturally sensitive, knowledgeable about HIV and skilled in case management and service planning. Knowledge of the Patient Protection and Affordable Care Act is strongly preferred.

Job Summary Specific to Case Management, HTN Grant

This position, under the direct supervision of the Access Health Tri-County Network Program Director in the Case Management Department, serves to provide simple navigation services to identified low-level need population. Enrolls qualifying network participants in federal insurance exchange and screens participants in the state Benefits Bank for community services. Performs administrative duties as needed by the grant project.

Essential Job Functions

  • Assist case managers with applications to benefits program and reimbursement sources including, but not limited to, Medicaid and Medicare, AIDS drug assistance applications, ACA Insurance marketplace plans, prescription assistance programs, charity applications, and other insurance applications.
  • Arrange transportation for patients through contracted providers.
  • Assist case management team with day-to-day clinical case management services to assigned patients.
  • Facilitate case coordination, which is non-duplicative and collaborative; be responsible for timely two-way communication between other agencies; and ensure continuous seamless case management.
  • Maintain strict confidentiality of all client data and observe all policies concerning release of information.
  • Performs other job duties and responsibilities as required.
Job Duties and Responsibilities for Positions in Case Mgmt, AHTN Grant
  • Reviews client's insurance status and ability to secure additional eligible benefits using The Benefits Bank quick check process.
  • Reviews referral of assigned or referred clients to determine clients' insurance needs and facilitates their access to health care insurance exchange.
  • Identifies and refers to additional community resources as needed.
  • Assist Program Director in developing and maintaining a network of human services and community resources that partner with AHTN to provide supportive services to clients.
  • Documents all findings, actions and plans using guidelines and standards of Access Health South Carolina.
  • Performs other job duties and responsibilities as required.
Job Duties and Responsibilities for Positions in the HOPWA Program
  • Provide day-to-day casework on assigned clients enrolled in the Housing Opportunities for People with AIDS program (HOPWA) to include completing applications, developing care plans, providing information about housing, and other aspects of case management.
  • Assists patients in identifying and obtaining community resources and financial assistance.
  • Assists with Ryan White fundraising opportunities and events.
  • Make payments to and payment arrangements with both landlords and utility companies to ensure that stable housing is maintained.
  • Develop and maintain a network of human services and community resources which can and will provide support and/or services to clients to decrease dependence on HOPWA STRMU and TBRA funds.
  • Facilitate case coordination, which is non-duplicative and collaborative; engage in two-way communication between Ryan White case managers, LAS, and other agencies to ensure continuous seamless case management.
  • Arrange transportation for patients through contracted providers.
  • Performs other job duties and responsibilities as required.

Licensing/Certification

Certified Application Counselor (CAC) preferred for positions in HIV Grant Program.

Education

Bachelor's Degree in Social Work, Psychology, Health Education or related field (Required)

Work Experience

Experience in case management or counseling, preferably in a public health setting or with high-risk populations. Related unpaid and internship experience accepted. For positions in HIV Grant Program, strongly prefer experience in a healthcare/benefits navigation setting. For positions in Case Management, HTN Grant, a minimum of two years of community experience (preferred)
