Primary Function/General Purpose of Position
The Human Services Assistant assists the case management team with the delivery of clinical case management services to assigned patient populations. Services include but are not limited to outreach, monitoring, service planning, financial resource planning, case coordination, advocacy, education, and follow-up directly related to the patient.
Job Summary Specific to HIV Grant ProgramThe Human Services Assistant assists the medical case management team with the delivery of clinical case management services to persons living with HIV and their loved ones through the provision of a comprehensive system of case management. Services include insurance and benefits navigation, monitoring, service planning, financial resource planning, crisis intervention and prevention, case coordination, advocacy, education, and follow-up directly related to the patient. The individual must be nonjudgmental, culturally sensitive, knowledgeable about HIV and skilled in case management and service planning. Knowledge of the Patient Protection and Affordable Care Act is strongly preferred.
Job Summary Specific to Case Management, HTN GrantThis position, under the direct supervision of the Access Health Tri-County Network Program Director in the Case Management Department, serves to provide simple navigation services to identified low-level need population. Enrolls qualifying network participants in federal insurance exchange and screens participants in the state Benefits Bank for community services. Performs administrative duties as needed by the grant project.

Licensing/Certification
Certified Application Counselor (CAC) preferred for positions in HIV Grant Program.
Education
Bachelor's Degree in Social Work, Psychology, Health Education or related field (Required)
Work Experience
Experience in case management or counseling, preferably in a public health setting or with high-risk populations. Related unpaid and internship experience accepted. For positions in HIV Grant Program, strongly prefer experience in a healthcare/benefits navigation setting. For positions in Case Management, HTN Grant, a minimum of two years of community experience (preferred)
