DescriptionSouthern Tier AIDS Program, Inc. is looking for a Housing Specialist, Health Homes to join our Binghamton team! This position provides confidential housing placement and referral services to chronically ill Health Homes eligible individuals, their families, and significant others in order to promote housing stability and improved medical outcomes.. The Housing Specialist, Health Homes reports directly to the Assistant Director of Client Services, Housing. ResponsibilitiesThe primary responsibilities and duties of a Housing Specialist, Health Homes include housing referral services to chronically ill health home eligible individuals receiving services from STCC, their families and significant others. This includes but is not limited to:Case conferencing with other agencies/service providers/medical staff, in order to ensure comprehensive care plan development and integration and the promotion of medical outcomes in conjunction with access to stable, affordable housing;Arranging appropriate housing service linkages; including but not limited to public assistance, Section 8, emergency rental assistance, emergency utility assistance, homelessness prevention initiatives;Engaging clients, as needed, in the conduct of an initial housing assessment, re-assessment, service plan development, follow-up interviews and evaluations;Monitoring medical outcomes data and verifying required data with medical providersMonitoring housing stability, as well as documenting ongoing outcome measures related to such and tracking goal attainment;Maintaining working relationships with housing/financial assistance providers, landlords and rental companies;Serving as a client advocate as necessary;Documenting progress toward goals and activities.BenefitsExcellent benefits package including: life insurance, 401(k) retirement plan, medical and dental insurance, flexible spending plan and generous time off (24 days annually + 11 paid holidays)Salary$18.00/hour working 35-hours per week, may vary depending upon experienceRequirementsEducation:Associates degree and one-year experience working in a human service or community-based organization or HS diploma and 3 years equivalent experience working in a human service or community-based organization.Experience:Experience working in the human services, health education, health or other related field, housing assistance background and experience working with any of the following populations; persons living with a chronic illness, a history of mental illness, homelessness and/or chemical dependence preferred.Excellent organizational, time management, and communication (verbal and written) skills, ability to work autonomously, good judgment, and record keeping skills are essential. Computer literacy required. Physical demands are those associated with general office duties: sitting, standing, light lifting. This individual will travel in the community (i.e. visiting with service providers, client home visits, etc.).This individual is responsible for meeting the transportation and insurability requirements of this job. A valid driver's license, clean driving record and reliable transportation is a requirement of this position.
