United Hospice has an extraordinary opportunity for a skilled Registered Nurse (RN). As Hospital Liaison you provide both clinical and administrative oversight at your assigned hospital location for United Hospice (UH).

Salary Range For This Position: $80,000 to $100,000.

• Your responsibilities include developing a hospice presence in the hospital, attending and facilitating rounds and care conferences, including, conferring with all care health care providers with the primary purpose of increasing UH admissions, and performing hospice admissions as needed.
• It is essential our employees have an understanding and commitment to the hospice philosophy of care.
• Collaborating with a diverse patient population, you must be able to communicate effectively, while sustaining a caring, kind, and professional disposition.
• You will be expected to maintain the highest level of confidentiality concerning patient and family information.
• Bi-Lingual (Spanish-English) is a plus! Seeking candidates who are flexible and enjoy working in a team atmosphere.

• Candidates must be adept at identifying hospitalized nursing home residents who meet eligibility criteria to arrange for or conduct admissions to UH prior to discharge.
• Must develop and maintain positive relationships with referral sources and promote exemplary customer service to physicians, facility staff, patients, and families.
• You will serve as an integral member of our clinical team to work in conjunction with the hospital case managers in reviewing the hospital daily census to identify appropriate patients for UH admissions.
• Admitting patients onto hospice as needed.
• CHPN highly desirable. If not certified, must successfully sit for exam within three (3) years of the start of employment.
• Daily access to a reliable automobile is required. A valid NY or NJ State Driver's License, ability to drive and driving record acceptable for agency insurance coverage are required.
