Job DetailsJob Location Stith Center: 791 Chambers Road, Aurora, CO 80011 - Aurora, CO Position Type Full Time: FTE 1 Education Level Bachelor's Degree Salary Range $20.93 - $31.37 Hourly Description Aurora Mental Health & Recovery (AMHR) is seeking a skilled and compassionate Hospital Liaison to facilitate seamless communication between AMHR's clinical teams and external psychiatric inpatient treatment facilities, including CSU, ATU, and psychiatric hospitals. This role requires a professional with strong communication skills, a solid understanding of mental health treatment systems, and experience with electronic health record documentation. Working under the adult intensive services division, the Hospital Liaison will collaborate closely with internal and external colleagues to ensure coordinated care for clients transitioning from inpatient settings to AMHR treatment teams. Responsibilities include coordinating referrals, scheduling appointments, and gathering essential clinical information to support effective client care. Schedule: Monday-Friday, 8:30 am - 5:00 pm or 9:00 am - 5:30 pm with a hybrid work arrangement. This allows for a mix of remote and in-office work, with the expectation of being in the office 2 days per week (Wednesdays & Thursdays) and working from home 3 days. Please note that this schedule may be adjusted based on client and community needs. Salary for this role. The starting wage is based on experience and company equity.Paid bi-weekly.
  • $20.93 - $31.37per hour
Bilingual Language Differential Pay. The rate depends on language requirements for the position and the candidate's fluency.
  • Employees who successfully pass a language assessment are eligible to receive bilingual differential pay. This differential is paid on a bi-weekly basis and is equivalent to 5% of the employee's regular base rate (hourly or salaried), with a maximum of $3,000.00 per calendar year. The annual maximum amount will be prorated based on full-time equivalency (FTE).
Essential Functions:
  • Receiving communication from referring treatment agencies via phone, email or fax.
  • Responding to referring individuals and agencies in a timely fashion, usually within the same day, to discuss referral needs.
  • Providing a level of care assessment and determination for purposes of uniting clients with the treatment team that will provide the level of care needed upon discharge from the external treating facility.
  • Monitoring the e-fax for clinical referral data, uploading the data to the clients electronic record, and forwarding the clinical referral data to the assigned treatment team.
  • Maintain a data tracking record with the referral information to include the day of referral and scheduled follow up appointment.
  • Working with assigned hospitals to coordinate referrals of clients from acute psychiatric treatment units to other facilities for more intensive and longer term services.
  • Interacting with the Colorado Access liaisons to coordinate treatment and meet the 7 days post hospital follow up engagement goal.
  • Maintain regular communication with Aurora MHC treatment teams in order to be fully aware of the internal referral processes used to set clients up for their needed services.
  • On occasion provide education to internal and external referring individuals to assist for a smooth and collaborative referral process.
  • Work closely with primary supervisor to problem solve referral issues as they occur. This needs to occur as close to possible in real time in order to expedite the successful referral to post-hospital and ATU services.
  • Coordinate the referral process for certified clients including assisting with setting up the paperwork referral components, scheduling doctor to doctor consultations for referral of certified clients, and working with the intensive services teams to coordinate the team assignment.
QualificationsRequirements:
  • Bachelors degree
  • Preferred: Masters degree in a clinical human services field
  • No experience needed. Strong interest in working in Mental Health arena.
  • Ability to comprehend and effectively respond to all client presentations and crisis situations.
  • Ability to document interactions with clients, other agencies and mental health professionals.
