Our staff appreciate autonomy, encouragement to voice opinions, and support from management and colleagues.About Our Hospice and Palliative Care Programs Our programs make a significant difference for patients with life-limiting conditions, offering community and facility-based services to ease the burden of illness and end-of-life care.Responsibilities Provide psychosocial support and social work services to hospice patients and families in inpatient, home care, or nursing home settings, including pre- and post-bereavement phases.Serve as a social work member of the hospice interdisciplinary team (IDG) and liaise between hospice staff and inpatient/nursing home staff.Develop social work plans of care, set goals, and make recommendations based on patient and family needs for direct admissions and residents.Create social work and discharge plans for patients admitted to inpatient facilities.Qualifications Graduate from an accredited School of Social Work.Minimum of one year of social work experience in a healthcare setting.Knowledge of Word and Excel preferred.New York State MSW licensure required.Valid NYS Driver's license preferred.Ability to work within an interdisciplinary team.Excellent organizational, communication, and listening skills.Proficient in inputting clinical data.Bilingual in English and Chinese (Mandarin or Cantonese) required.#J-18808-Ljbffr
