WEEKEND TRACK - Hospice - Admissions&Central Intake RN

Every Friday 4 hour days, Every Saturday & Sunday 10 hour days (8a-6p) Eligible for Full-Time Benefits

The Community Hospice has a rewarding opportunity for a full-time RN to work every Friday/Saturday/Sunday as a part of our Admissions and Central Intake Teams in Hospice Home Care.

What you will do:

Admits patients to home care hospice services

Responsibilities:

* The RN coordinates with the patient's community physician and our hospice medical directors to confirm eligibility for hospice services.

* Goes to patients' homes to complete the initial assessment of patient/family needs and begins implementation including ordering medications, medical equipment, supplies and support from our interdisciplinary team which includes social workers, chaplains, volunteers, complementary therapists, home health aides, and more!

* Ensures a smooth hand-off to the Hospice Case Manager.

* Intake responsibilities consist of working in the Hospice office on 445 New Karner Road in Albany assessing eligibility for Hospice services, processing and scheduling admissions.

* Option to cross train in Admissions.

* Corporate office is 445 New Karner Road-Albany NY with satellite offices in Catskill and Saratoga NY. Admissions are across 8 counties: Columbia, Greene, Albany, Rensselaer, Schenectady, Saratoga, Washington. Your region is considered in patient assignment.

What you will need:

* NYS RN Licensure

* ASN required, BSN preferred

* 1 year RN experience required, 2 years' RN experience with recent acute and/or hospice experience is preferred.

* Reliable vehicle and driver license

* Must be able to communicate effectively with patients, physicians, and peers.

* Must be able to document key information concisely and accurately

* Strong computer skills with the ability to navigate through several EMR platforms

* Strong organizational and time management skills

SUPPORTED BY A HOSPICE RN SUPERVISOR in the office 8am to 4pm Saturday and Sunday and an OnCall RN Supervisor after hours, so assistance and guidance is only a phone call away!

Weekend Track Rate: $51.30/hr

Pay Range for non-track/non-per diem Part-Time and Full-Time: $35.00-$47.75

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
