As a faith-based, non-profit leader in home health and hospice care, we're shaping the future of healthcare with innovation, heart, and a mission to serve.🌟 Your Role: Hospice RN Case ManagerAs a Hospice RN Case Manager, you'll provide one-on-one care to patients with life-limiting illnesses, helping them live with dignity, comfort, and peace. You'll lead the care team, coordinate services, and support patients and families through every step of their journey.💼 What You'll Do

• Develop and manage individualized care plans
• Provide symptom management and emotional support
• Educate patients and families on end-of-life care
• Coordinate with physicians and interdisciplinary teams
• Supervise Hospice Aides and LPNs
• Document care accurately using point-of-care technology
• Participate in IDT/IDG meetings and case conferences
• Ensure timely communication and care coordination

✅ What You'll Need

• Graduate of an accredited nursing program
• Active RN license in the State of Iowa
• Minimum 1 year of professional nursing experience (acute care preferred)
• Hospice or home care experience preferred
• Strong communication and interpersonal skills
• Valid driver's license and reliable transportation
