Description:?Full time Hospice Liaison RN -Albany The Community Hospice has a rewarding opportunity for an RN to work as a part of our Hospice Team serving as Liaison in local Nursing Homes . What you will do:The Liaison RN evaluates patients for eligibility and interest in hospice services, describes the support and care available from hospice along with the benefits of hospice care, and coordinates with the patients community physician, facility, and our hospice medical directors to confirm eligibility for hospice services. Admits patients to hospice in the nursing home or ensures a smooth handoff to the hospice RN admitting the patient/family to services.
