Position Overview
We are seeking a compassionate and skilled Hospice Registered Nurse to deliver high-quality end-of-life care to patients and their families. This role ensures timely, coordinated, and holistic hospice services in alignment with the interdisciplinary care plan and physician directives.
Essential Job Functions:
Conduct regular and after-hours home visits to assess patient/family needs and document care plans, interventions, and outcomes.
Collaborate with the interdisciplinary team to develop and revise care plans, ensuring continuity across providers and settings.
Supervise Hospice Aides and LPNs, ensuring care aligns with established plans and documentation standards.
Participate in on-call rotations, long-term care coordination, and facility care plan meetings.
Educate patients, families, and caregivers on hospice philosophy and services.
Maintain compliance with Medicare, Joint Commission, and agency standards.
Support community outreach and program growth initiatives.
Manage medical supplies cost-effectively and participate in quality improvement activities.
Ensure timely, accurate documentation and uphold privacy and security protocols.
