Hospice Patient Care Leader - Gentiva Hospice : Job Details

Join our dedicated team as a Hospice Patient Care Leader, RN. In this vital role, you will report to the Executive Director, Administrator, or Senior Patient Care Manager. Your primary responsibility will be to ensure the delivery of exceptional care within a high-performing hospice program. You will manage, oversee, and direct patient care activities, while also mentoring and developing your team to adhere to company policies, state regulations, and industry standards.

• Manage and oversee rescheduled, declined, missed, and reassigned visit requests, prioritizing based on clinical judgment.
• Respond professionally and compassionately to phone referrals and inquiries, triaging medical concerns within your scope of practice.
• Prepare for and facilitate the Interdisciplinary Group (IDG) meetings as assigned, including leading meetings, presenting patient reviews, coordinating minutes, and reviewing patient charts to monitor any changes in patient condition or care plans.

About You:

• A minimum of three years of direct patient care experience and two years in a hospice or home-based care setting.
• A graduate of an accredited nursing school; a bachelor's degree in nursing is preferred.
• Current Registered Nurse licensure in your state of residence.
• Knowledge of hospice principles and practices, including compliance with regulations and best practices (Medicare, Medicaid, JCAHO, ACHC).
• Ability to understand and respond to the unique needs of hospice patients and their families from diverse backgrounds.
