• Deliver comprehensive palliative care in an outpatient and home-based setting.
• Perform thorough patient assessments, develop and implement individualized care plans, and provide symptom management.
• Prescribe medications, order and interpret diagnostic tests, and provide counseling and education to patients and families.
• Collaborate closely with a multidisciplinary team, including physicians, nurses, social workers, chaplains, and other healthcare professionals.
• Support patients physical, emotional, spiritual, and social needs during some of the most critical times in their lives.
• Participate in quality improvement, education, and research initiatives to enhance palliative care services.
