Hospice LPN, Continuous Care, NIGHTS - Accentcare : Job Details

Deliver skilled nursing care to patients and families in accordance with the interdisciplinary plan of care, including assessment, planning, implementation, and evaluation Coordinate with the care team, physicians, and external partners to ensure high-quality, continuous care, while keeping patient records updated and accurately reflecting changes in status Educate and support caregivers on medication management, personal care, mobility assistance, end-of-life preparation, and other aspects of patient care Serve as a key communicator with the Director of Clinical Operations and interdisciplinary group (IDG) to address any unusual or complex patient or family issues Provide compassionate support during critical moments, including at the time of death, and contribute to 24/7 care coverage as needed Participate in team meetings, training programs, compliance efforts, and additional duties or projects within the scope of professional licensure Our Investment in You Caring for others starts...Continuous, Hospice, Healthcare, Patient
