Join our team as an experienced RN with hospice case management expertise. Manage a caseload of 15 patients, conducting 4-5 visits per day based on area and patient acuity. Complete charting on the same day. Perform all service level nursing, including routine, GIP, continuous, and respite care. Conduct admissions, SIA, and death visits. Requirements
- Minimum 1 year of hospice case management experience
- Acute/Med Surg experience preferred
- BLS (AHA) certification required
- Active RN license in Washington; Oregon license encouraged
- Reliable vehicle for a 60-mile service radius (mileage reimbursed at IRS rate)
- Clean background check and drug screening
- Influenza, TB, and Covid vaccination records
- Valid driver's license and auto insurance
