Hospice Care Aide - Per Diem/PRN - Gentiva Hospice : Job Details

Are you a compassionate Hospice Aide/CHHA eager to provide hands-on care to those in need? Your Role as a Hospice Aide:

• Deliver direct personal care to terminally ill patients
• Assist with hygiene, grooming, and toileting needs
• Facilitate safe mobility using walkers and wheelchairs
• Support meal preparation, feeding, and light housekeeping duties
• Monitor and report any changes in patients' physical or mental health
• Provide emotional and psychological support to patients and their families
• Maintain accurate documentation of all care provided
• Engage in team meetings and participate in Quality Improvement (QI) initiatives
• Adhere to infection control and safety protocols consistently

What You Bring:

• Valid CHHA license in your state (Required)
• Current CPR certification (Required)
• Driver's license and reliable transportation with insurance (Required)
• Minimum six months of direct patient care experience (Preferred)
• Comfortable working in home care and hospice environments
• Demonstrated compassion and patience
• Ability to work independently and adapt to changing patient needs

Preferred Experience (Not Required):

• Experience in home health, hospice, palliative care, or oncology
• Previous nursing roles in hospitals, geriatrics, or long-term care
• Exposure to end-of-life care, wound management, or admissions support
