Uvalde Memorial Hospital's Hospice Program is seeking a dedicated PRN Hospice Patient Care Assistant to support terminally ill patients and their families in Uvalde and surrounding areas. Position: PRN Hospice Patient Care Assistant Location: Uvalde, Texas Department: Hospice Schedule: PRN (as needed) Status: Non-Exempt Job Summary: Under the supervision of a Registered Nurse Case Manager, the Hospice Patient Care Assistant provides personal care services and emotional support to terminally ill patients in their homes or care facilities. This position plays a key role in implementing the Interdisciplinary Team Care Plan, helping ensure patients receive comfort, dignity, and respectful end-of-life care. Key Responsibilities:
  • Provide personal care and assist with activities of daily living
  • Perform duties within CNA scope of practice including bathing, dressing, toileting, and mobility assistance
  • Support safe patient ambulation, transfers, and use of durable medical equipment
  • Offer compassionate care and emotional support to patients and caregivers
  • Observe and report changes in patient condition to the RN Case Manager
  • Assist caregivers in providing appropriate personal care
  • Communicate clearly with team members and participate in hospice-related meetings, in-services, and performance improvement activities
Qualifications:
  • High school diploma or equivalent
  • Certified Nurse Aide (CNA) in the State of Texas - Required
  • Current CPR certification - Required
  • At least one year of CNA experience - Required
  • Prior hospice experience - Preferred
  • Valid driver's license, reliable vehicle, and current auto insurance - Required
  • Bilingual English/Spanish a plus
Physical Requirements: This position involves standing, walking, bending, kneeling, reaching, lifting, and the use of assistive devices. Must be able to occasionally lift heavy objects and respond to patient needs in home and facility environments. Why Uvalde Memorial Hospital?
