Job Type Full-time Description Hours for this position are flexible with either 5 - 8 hour shifts, 3 - 12 hour, or 4 - 10 hour shift, with one Saturday per month. Position is located in Rockford, IL with travel throughout our service area. The Admissions Nurse is responsible for evaluating patients for hospice eligibility and provides timely and effective initial nursing assessments of patients (and families) admitted for hospice services. This assessment includes a thorough explanation of the Hospice philosophy, available services, and Advance Directives, obtains the appropriate consents, conducts an initial physical assessment of the major body systems, and establishes the initial Plan of Care for the patient.

  • Assesses patients for hospice eligibility and provides accurate and timely documentation, including pertinent and complete data in the electronic medical record, according to documentation guidelines.
  • Explains the Hospice philosophy, available services, and Advance Directives,
  • Obtains the appropriate consents, conducts an initial physical assessment of the major body systems, and establishes the initial Plan of Care for the patient.
  • Obtains referral information from intake team.
  • Facilitates communication from intake or referring partner to insure prompt admission to hospice program.
  • Explains the hospice program of care and obtains appropriate signed consent for care from the patient/family.
  • Performs clinical record review and patient assessment to ensure hospice LCD eligibility criteria guidelines are met for admission to hospice program.
  • Performs admission within the required time frame after referral to insure prompt admission to the hospice program.
  • Accurately completes a thorough initial assessment to identify the patient/family's immediate needs, including but not limited to supplies, equipment, services and coordinates the provision of these services.
  • Accurately and completely documents patients' condition in the EMR system according to policy, including but not limited to, Visit Reports, Care Plans, and logs Plan of Care & IDG prep notes (if applicable)
  • Initiates initial plan of care in response to identified problems, concerns and needs to insure palliation of symptoms and safe, competent and continuity of care.

The expected base salary range for this role is $78,000 - $108,000. However, final base pay will be carefully determined based on several factors, including education, experience, knowledge, skills, and abilities, as well as internal equity and alignment with market data. Requirements Education: RN license, Minimum Associates Degree in Nursing; BSN preferred. Training and Licensure: Current Professional Registered Nurse license issued by the Illinois Board of Nursing. CPR Certification required. Certified Hospice and Palliative Nurse (CHPN) preferred. Knowledge/Skills/Ability: Knowledge of Local Coverage Determination (LCD) required. Experience: One year of Medical/Surgical, Oncology, or Hospice nursing experience preferred. Salary Description based on experience and education
