Position TitleHome Hospice Registered NurseDays - Full TimeOlathe Medical Pavilion DPosition Summary / Career Interest:The Hospice RN utilizes the nursing process to coordinate the interdisciplinary approach to patient care for her/his team of patients. He/she promotes continuity through communication of the patient's plan, condition and progress toward outcomes. The Hospice RN participates in and demonstrates effective education and support for patients and their families to meet their physiological, psychological, and spiritual needs. He/she practices within the boundaries of the Kansas Nurse Practice Act, specialty standards, and organizational/departmental policies and procedures.
Responsibilities and Essential Job Functions- Consistently meets work schedule.
- Demonstrates critical thinking utilizing all aspects of the nursing process in all aspects of patient care (Assessment, Diagnosis, Plan, Intervention, Evaluation).
- Maintains a safe environment for patients, caregivers and staff. Specific emphasis on education of O2 safety and smoking precautions to our families.
- Teaches patient and family caregivers appropriate medication for palliation, according to plan of care. Administers medications which provide optimum comfort and monitor effectiveness of medications. Notifying physician of needs to change meds for unmet goals or drug interactions.
- Completes clinical procedures effectively (per OMC policy), utilizing products and equipment appropriately.
- Effectively assesses and manages pain and other end of life symptoms.
- Provides appropriate patient/family teaching regarding end-of-life care, using resources available.
- Delegates appropriately according to task situation, level of expertise and functions as an independent and supportive team member.
- Coordinates patient care though effective and professional communications with physicians, nursing and ancillary staff in timely manner. Provides updates to on-call nurse and makes arrangements with other staff to cover a visit if needed. Participates in weekly IDT meetings. Documents accurately and timely according to agency policy.
- Establishes therapeutic and caring interpersonal relationships with patients/families. Recognizes need to ask for help in establishing boundaries.
- Demonstrates knowledge of and involvement in ongoing performance improvement activities.
- Professional role model; acts as preceptor/mentor for new employees and students. Participates in community education and memorial services. Takes a leadership role in building and maintaining an environment where all team members thrive.
- Identifies areas for professional growth, i.e. sets goals and implements plan, participates in professional association. Seeks out additional learning for professional growth.
- Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.

Required Education and Experience- Associate Degree Nursing
- Previous Healthcare Experience
Preferred Education and Experience- Bachelor Degree Nursing
- 1 or more years Hospice and/or Home-based Healthcare Delivery
Required Licensure and Certification- Licensed Registered Nurse (LRN) - Single State - State Board of Nursing upon Hire OR
- Licensed Registered Nurse (LRN) - Multi-State - State Board of Nursing upon Hire
- CPR/ AED/ BLS - Other BLS within 14 days
