Key Responsibilities:

• Conduct comprehensive assessments to formulate personalized care plans.
• Administer medications, treatments, and nursing procedures as necessary.
• Monitor clients' health status (vital signs, progress) and adjust care plans accordingly.
• Educate clients and families on disease management, medication usage, and home safety protocols.
• Execute advanced nursing procedures, including wound care, IV therapy, and catheter management.
• Document all care provided, ensuring compliance with regulatory standards.
• Uphold adherence to state and federal healthcare regulations.
• Provide emotional support and guidance to clients and their families.
• Stay informed on nursing best practices and engage in professional development activities.

Qualifications:

• Active RN License in the state of New York.
• 1-2 years of nursing experience preferred (home health, geriatrics, or similar settings).
• Strong clinical skills and ability to perform nursing procedures independently.
• Excellent communication skills with a proven ability to build rapport with clients and families.
• Self-motivated and organized, capable of working independently in a home care environment.
• Reliable transportation and a valid driver's license.
• CPR and BLS certification (required).
• Experience with electronic health records (EHR) systems (preferred).
