Home Health Physical Therapist Join our dedicated healthcare team as a Home Health Physical Therapist where you will be responsible for providing top-notch care directly to patients in their homes. This role demands a dynamic professional who can assess patient needs, develop and modify care plans, and ensure continuous improvement in patient outcomes through effective collaboration with health care teams. Salary: $95,000.00 - $110,000.00 per year Essential Duties and Responsibilities:
- Conduct thorough evaluations of patient condition and create individualized plans of treatment including both short and long-term goals.
- Monitor patient progress and respond to treatment, providing timely updates and adjustments to the care plan as necessary.
- Provide expert instruction to patients, caregivers, and other members of the health team on handling patient disabilities and recovery processes.
- Maintain ongoing communication with physicians and healthcare team members to ensure a cohesive treatment approach.
- Prepare and educate patients and caregivers for post-discharge care to enhance recovery and independence.
- Report significant patient, family, or physician issues to the Clinical Director to ensure all stakeholders are informed and engaged in the treatment process.
Minimum Position Qualifications:
- Current state licensure as a Physical Therapist required.
- Must possess a valid CPR certification.
- Valid driver's license and proof of automobile liability insurance.
- Proven ability to document and communicate medical information effectively.
- Knowledgeable in professional therapy services principles, theories, and practices.
- Skilled in establishing and maintaining cooperative relationships with patients, families, and medical staff.
Preferred Qualifications:
- More than 1 year of experience in a home health setting is highly desirable.
- Advanced certifications relevant to physical therapy and home health care.
- Experience in handling complex patient cases with multiple treatment requirements.
- Demonstrated ability to lead and educate other health care professionals and staff.
