Position Details:

• Status: Full Time or Part-Time (Minimum of 5 visits or 10 hours per week for part-time)
• Compensation: $75-$150 per visit, plus travel and mileage reimbursement
• Average Earnings: $97k - $120k annually for full-time therapists

Your Role:

• Deliver home-based physical therapy to patients recovering mobility, strength, and functionality
• Collaborate with a multidisciplinary home health team focused on individualized patient care
• Conduct patient assessments, document progress, and adjust care plans as needed
• Guide patients through therapeutic exercises to alleviate pain and promote mobility
• Educate patients and families about care plans, therapeutic goals, and self-care techniques

Requirements:

• Graduate from an accredited Physical Therapist program and active PT license in Colorado
• Minimum of 1 year of physical therapy experience, preferably in home healthcare
• Current BLS/CPR certification
• In-depth knowledge of home health regulations
• Exemplary skills in clinical judgment, interpersonal communication, and compassion
• Must pass a background check and drug test (testing for THC required)
• No Covid vaccination required
