As a Full-Time Home Health Physical Therapist, you will conduct patient visits in the Highland county area, including Sebring. You will be responsible for:

• Coordinating and directing the care of home health patients requiring physical therapy.
• Managing a caseload while adapting to the changing needs of patients and families.
• Conducting comprehensive assessments and formulating individualized care plans in collaboration with patients, families, and physicians.
• Reassessing patients periodically to evaluate care effectiveness and make necessary adjustments.
• Documenting all activities, findings, and patient reactions to treatment for continuity and quality of care.

To be successful in this role, you should have:

• A Bachelor's or Master's Degree in Physical Therapy
• At least 1 year of relevant clinical experience
• A valid Physical Therapist license in your state of practice
• A valid Driver's License and current car insurance
• An active American Heart Association BLS certification

Preferred qualifications include:

• Doctorate in Physical Therapy
• Experience in a Medicare-certified home health agency
• Oasis Specialist-Clinical (COSC) certification
• Home Health Care-Manager Certification
