Join us as a Patient Care Coordinator and play a critical role in ensuring that patients receive timely, accurate, and compassionate support during their care journey. We serve thousands of patients across the state and are known for our commitment to client-centered service. What You'll Do:
  • Manage and track key patient documents, including face-to-face forms for physician signatures
  • Gather hospital records and key paperwork such as H&P, physician orders, and ROC notes
  • Make pre-admission, follow-up, and discharge calls to ensure accuracy and continuity
  • Monitor hospitalized clients and notify liaisons of discharges for timely care coordination
  • Provide administrative support that helps our nurses, therapists, and care teams do their best work
Qualifications:
  • High school diploma required; college degree preferred
  • At least one year of experience in a healthcare or fast-paced office setting
  • Strong verbal and written communication skills, with a phone presence that builds trust
  • Typing speed of 50+ WPM and comfort with navigating electronic medical records
  • Familiarity with medical terminology and home care processes is a plus
