Home Health CNA - Aide Services - Health First : Job Details

Job Requirements Job Description. The Home Health C N A is responsible and accountable for providing Quality / No Harm, Customer Experience, and Stewardship by performing or assisting with basic patient care functions and providing appropriate age-specific therapy services in accordance with the plan of care established by the Case Manager. Primary Accountabilities. Assists the patient with personal care activities following the plan of care established by the Case Manager. Performs HHA/ CNA skills as taught/directed by the Case Manager. Documents services in an appropriate and real time manner utilizing Telephony. Notifies Case Manager with changes in patient condition. Documents changes on narrative note and submits with Patient Visit Log for scanning. Transports blood/urine specimens to appropriate laboratory facility. Assist in Oximetry and Telehealth by providing patient education during the installation and removal process of equipment.
