Home Health Care Coordinator - Manpower San Diego : Job Details

We are seeking a compassionate and organized Home Care Coordinator (HHA) to join our team. This is a key position supporting the Home Care Coordination RN Manager and the interdisciplinary team (IDT). In this role, you'll ensure high-quality unskilled home-based care for participants by coordinating the delivery of meals, caregiver services, and durable medical equipment (DME).

You'll be a proactive communicator, advocate, and problem-solver, ensuring care is person-centered and aligned with each participant's preferences and health needs.

Key Responsibilities:

• • Conduct home safety evaluations (pre-enrollment, annual, and change of address)
• • Coordinate IDT-approved services including: Meal delivery, Caregiver services, Durable Medical Equipment (DME) and incontinence supplies
• • Document service delivery in participant medical records (EMR)
• • Track, upload, and manage care documentation in compliance with DHCS and CMS guidelines
• • Monitor service delivery and report gaps in care to the IDT
• • Support nurses in coordinating home services
• • Conduct monthly visits for homebound and participants
• • Deliver supplies and equipment as needed
• • Communicate with participants, families, vendors, and the care team to ensure seamless, compassionate care

Qualifications:

Certified Home Health Aide (HHA) – California Department of Public Health

Certified Nursing Assistant (CNA)

CPR and First Aid certified – American Heart Association

Valid California Driver's License

Minimum 1 year experience working with frail or elderly populations

Strong communication, coordination, and documentation skills

Proficient in Microsoft Outlook, Word, and Excel
