- Provide Direct Care – Assist clients with bathing, dressing, toileting, transferring, simple wound care, and other personal care tasks as directed by a nurse or therapist.
- Maintain a Safe Home Environment – Perform client-directed light housekeeping tasks, ensuring a clean and comfortable living space.
- Document & Communicate Effectively – Accurately record client care per policy and report updates or concerns to the supervising nurse or therapist.
- Enhance Social Well-Being – Establish trusting relationships with clients and their families, providing compassionate companionship and emotional support.

This Job Also Requires
- Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification required.
- Ability to follow care plans and work independently in a home setting.
- Knowledge of safe lifting and transfer techniques.
- Experience in home health, long-term care, or personal caregiving is preferred but not required.
- Valid driver's license and access to an insured personal vehicle for home visits.
- Physical capability to perform all essential job functions, including assisting with client mobility and personal care needs.
- Ability to pass state mandated background check.
- Ability to read, write, and speak English to ensure effective communication with clients, staff, and families.
