Facility: Seagate Rehabilitation and Nursing CenterPosition: Home Health Aide (HHA)Location: Brooklyn, New YorkShift: Full-Time | All three shifts (6:00 AM – 2:00 PM, 2:00 PM – 10:00 PM, 10:00 PM – 6:00 AM)Salary: $20/hour

About the Role:Seagate Rehabilitation and Nursing Center is seeking a compassionate and dedicated Home Health Aide to escort residents to and from clinic and dialysis appointments while ensuring their safety, comfort, and well-being. This role requires a proactive and professional individual who can represent our facility with integrity both on and off-site.

Key Responsibilities:

  • Escort residents to and from clinic and dialysis appointments.

  • Arrive at work 1 hour prior to scheduled clinic appointments.

  • Be in full nursing uniform with ID badge visible at all times.

  • Assist residents with preparing for appointments.

  • Provide residents with their out-on-pass snacks.

  • Monitor residents to prevent falls or altercations; intervene immediately to prevent incidents.

  • Report all incidents or accidents to the Clinical Coordinator or ADNS immediately.

  • Obtain appropriate reports and paperwork for the resident's clinic visit.

  • Verify each resident's DNR/DNI status and ensure non-hospital DNR forms are available when required.

  • Act as a proactive advocate for residents at clinic visits, communicating any issues to the facility promptly.

  • Represent the facility professionally, adhering to all policies, procedures, and abuse laws.

  • May be required to attend more than one clinic visit per day depending on facility needs.

  • Coordinate with the clinic coordinator regarding unit assignments before or after clinic visits.

  • Report directly to the Clinic Coordinator or ADNS.

Requirements:

  • High school diploma or equivalent preferred.

  • Current NYS HHA certification required.

  • CNA certification preferred.

  • Minimum 1 year of experience with EMR (Sigma preferred).

  • Prior nursing home experience preferred.
