POSITION SUMMARY: Provide intermittent nursing services to patients in their homes under a collaborative care plan established with the patient's physician and follow agency policies and procedures. Responsibilities include admitting clients, establishing care plans, and referring to other disciplines and agencies as needed.

EDUCATION/CERTIFICATION: Graduated from an NLN-approved nursing program (Associate or Baccalaureate degree). Must have a current Registered Nurse license in Connecticut.

EXPERIENCE: Minimum of one (1) year of recent medical/surgical nursing experience. Additional experience in community health nursing is preferred.

COMPETENCIES: Knowledge of nursing theory and practice, agency policies, medical equipment, safety precautions, and patient care documentation. Strong skills in assessing patient needs, problem-solving, and communication are required.

ESSENTIAL DUTIES and RESPONSIBILITIES:

  • Admit/readmit patients and provide nursing services in compliance with agency policies and regulations.
  • Conduct comprehensive assessments: physical and psychosocial.
  • Develop and implement a care plan in collaboration with patients/families.
  • Evaluate patient safety continuously.
  • Maintain effective coordination of care among all involved services.
  • Document all services in clinical records according to regulations.
  • Manage work assignments effectively, ensuring timely visits.
  • Plan and evaluate Home Health Aide care plans.
  • Develop and maintain professional skills through ongoing education.
