HOME CARE PROVIDER Monadnock Worksource is looking for Home Care Provider in the Keene area to join our Adult Foster Care Team, for a young man who prefers to stay active, needs help planning and building life skills. Currently has an established staffed day program Monday-Friday and planned respite available as well. Responsibilities: Assist individuals with daily living skills, including personal hygiene, routines, housekeeping, nutrition, and cooking. Develop and implement routines that promote skill-building, community participation, and relationship-building. Provide transportation to appointments, shopping trips, and community activities. Manage schedules for medical, dental, and other necessary appointments. Coordinate medication administration and ensure compliance with health needs. Requirements: Valid driver's license and auto insurance, Home or renters' insurance. Ability to pass criminal, DMV, DCYF, and BEAS background checks. Your home must meet State Certification Requirements. Benefits: A generous bi weekly stipend and support from a great team. Individuals have day programs 6 hours each weekday.
