What You Will Do

• Practice independently within the community as part of an interdisciplinary care team.

• Conduct comprehensive assessments of patients to identify their needs and develop tailored treatment plans.

• Deliver personalized rehabilitation therapy to patients in their homes or care facilities.

• Continuously evaluate patient needs and adjust care plans accordingly.

Qualifications

• A minimum of one year of physical therapy experience or successful completion of the probationary period as a VNS Health Rehab Intern.

• New York State License and current registration in Physical Therapy.

• A valid driver's license or NYS Non-Driver photo ID card may be required.

Pay Range per Visit:

• If applying for areas other than the Bronx, the hiring range is: $70 - $75 per visit.

• If applying for the Bronx, the hiring range is: $90 - $95 per visit.

Pay Range USD $85,000.00 - USD $127,500.00/Yr.
