Clinical Excellence/ Care Delivery - Home Care Aide Services - Provide physical support to assist clients to perform daily living activities such as bathing, dressing, toileting, standing, walking, or light exercise. - Reminds clients to take medications. - Uses safety measures to prevent incidents and instructs clients in home safety measures. - Notifies RN on same day of changes in client's condition and documents notification. - Demonstrates competency in all skills assigned by RN. - Gives clients information on community resources and notifies supervising RN of action. - Encourages clients to complete tasks as able for themselves. Homemaker Services - Completes assigned household tasks in courteous, efficient manner. - Complies with client's needs in completing tasks according to their instructions. - Observes other tasks that could contribute to client's safety and makes suggestions to RN regarding care plan.
