High Fidelity Family Support Partner - Youth Villages : Job Details

Educate families, providers and the community about the Wraparound process. Provide education and assistance in navigating the child serving systems (child welfare, education, behavioral/mental health, juvenile justice, school, primary care, etc.)Facilitate and assist in identifying formal and informal community resources, support, coaching, and training for the parent(s) / guardian(s)Empower family to make their own decisions and take control of their life. Assist family in learning how to problem solve, set goals, make decisions, and advocate for themselves. Assist the youth and parent/guardian to articulate the youth's strengths, needs and goals. Schedule at least one face-to-face meeting with family prior to the first Wraparound Child & Family Team meeting to aid the youth and parent(s)/guardian(s) in preparing for the first Wraparound Child & Family Team meeting.
