Job SummaryThe Healthcare Assistant for Clinical Services in the Division of Community Living Services is responsible for scheduling healthcare appointments for individuals with Intellectual Disability, some of whom may have a secondary diagnosis of mental illness. This position involves coordinating medical appointments, maintaining communication with the treatment team, and ensuring the accuracy of medication records.Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.RequirementsEssential Responsibilities, include but are not limited to:
- Schedule and Coordinate Medical Appointments: Schedule and coordinate members' medical appointments, including routine annual check-ups and appointments based on medical needs.
- Maintain Communication: Keep the treatment team informed of any changes in medical status, including emergency room visits, hospitalizations, routine appointments, and specialty appointments. Document these updates as necessary.
- Specialty Providers: Schedule and coordinate appointments with specialty providers for essential screenings and treatments.
- Quality Assurance: Assist with monthly Quality Assurance by reviewing bubble pack returns and ensuring medication records are accurate.
- Medication Error Reports: Assist in generating Medication Error reports for identified errors and forward them to the CILA nurse for review and signature.
- Medication Check-In: Daily check-in of medications from the contract pharmacy.
- Medication Refills: Order medication refills as requested by members.
- Face Sheet Updates: Review and ensure members' face sheets are current, forwarding necessary updates to the QIDP.

Qualifications:
- LPN preferred, Medical Assistant, Associate Degree, or three years of experience in the healthcare field.
- Excellent written and verbal communication skills. Strong decision-making and problem-solving abilities.
- Excellent computer and organizational skills.
- Ability to work independently.
- Valid driver's license and approval through the Agency's insurance carrier to drive Agency's vehicles.
- Must pass a Criminal Background Check and have a negative Healthcare Workers Registry report if applicable.
