POSITION SUMMARY: Perform the duties required to promote and explain Health Center Services, programs and health care benefit plans. Assist community residents and patients with Health Center Appointments and enrollment into pertinent health care plans and programs.POSITION QUALIFICATION REQUIREMENTSEducation: High School Diploma or Equivalent required. Experience: Requires one (1) year of community health, neighborhood outreach or related field.One (1) year experience of customer service experience. Skills and Abilities Strong verbal and written communication skills.Good organizational skills and interpersonal skills.Good computer skills including the use of word processing and e-mail software.Transportation Requirement: Must possess a valid driver's license and automobile with insurance coverage as required by the State of Missouri.License(s)/Certification(s) Required: Must participate in Exchange Marketplace training and successfully pass certification exam.CPR Certification required.Essential FunctionsContacts and recruits to promote the Health Center services.Submit patient appointment requests to the appointment staff. Track and monitor kept appointments. Recruits and assists with enrollment in the MO Health Net Programs, Presumptive Eligibility, DOSA, Temporary Medicaid and utilizes the EMOMED -MO Health Net provider system - for approval status.Recruits for participation in Community fairs, Outreach drives, Chronic Disease services, Mammography screenings, special Dental activities and WIC.Distributes health center materials (fliers) in neighborhood sites, such as stores, homes, and schools.Enters data into the NEXTGEN system.Keeps daily logs of phone calls, home visits, other contacts and monthly statistical reports.Conducts surveys and assists with monitoring patient/client flow in waiting area as required. Performs other duties as assigned.Marginal FunctionsMakes presentations on programs when needed.Attends other Affinia Healthcare and community events as assigned.
