  • Represent GMHC at public events, meetings, and partner sites to increase program visibility and build trusted relationships.
  • Enrollment & Client Advocacy: Assist individuals in enrolling in new health insurance plans or renewing existing coverage via the New York State of Health marketplace.
  • Support clients in gathering necessary documentation to complete health insurance applications or updates.
  • Provide clear, culturally responsive education to clients on plan options, benefits, and how to use their insurance.
  • Facilitate referrals through the Program Coordinator for individuals seeking additional services or support.
  • Documentation & Reporting: Accurately document all enrollment activity, services, and deliverables using the NYSDOH portal and internal tracking systems, including TREAT.
  • Participate in internal program reviews, data quality checks, and ensure compliance with funder reporting standards.
  • Assist in the collection of client feedback through surveys to support program evaluation and quality improvement.
  • Maintain accurate enrollment and documentation records across a variety of benefit programs, including NYSOH Marketplace, Medicaid, Medicare, ADAP, and COBRA, ensuring that all contract deliverables are properly tracked.
  • Ensure timely and accurate documentation of all client interactions, with data entered into the NYSDOH portal and internal systems (e.g. TREAT) within required timeframes to meet program and funder reporting standards.
  • Program Support & Collaboration: Maintain relationships with external partners and fixed enrollment sites to support program accessibility and success.
  • Participate in team meetings, staff trainings, and collaborative planning to improve service delivery.
  • Adhere to all agency and funder policies including documentation, confidentiality, communication, and conflict resolution protocols.
