Employee engagement activities and 403B match after 12 months of service.Job Summary:The Health Information Specialist will maintain, update, monitor and secure the medical records for the organization. The Health Information Specialist will be responsible for compiling data and analyzing necessary compliance metrics. The Health Information Specialist will be responsible for collecting, analyzing, and indexing patient care documentation into the electronic medical record as well as processing all medical record requests. The Health Information Specialist will be responsible for processing provider orders, faxing, and timely follow-up.RequirementsEducation/Qualifications:High school diploma or equivalent.Minimum of one year of experience in a position using medical secretarial skills.Certification, accreditation or associate degree in healthcare administration, medical records, or a related field preferred.Medical terminology certificate preferred.Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.Key Responsibilities:Audits for compliance and indexes accurately into the EMR all patient-related forms Audits patient admissions, recertifications, transitions in care, level of care changes, and deaths/discharges/revocations/transfers for compliance to organization, state, federal, and insurance regulationsProcesses, sends, and resends all medical orders and plans of care to the appropriate provider within expected timeframe and in accordance with all appropriate regulationsAudits for compliance and indexes signed medical orders and plans of care into the EMRManages new attending provider signature logs to ensure complianceAudits external attending and internal providers' licensure, PECOS enrollment, and any other eligibility criteria within expected timeframeUtilizes external software to maintain fluid patient data exchange necessary for continuity and quality of care including but not limited to Vynca, Epic, and NC DaveParticipates in medical records phone line rotation to appropriately respond to all internal and external health information related requests or concernsProvides direction and monitoring of work for office volunteers to ensure accuracy and completion of duties assignedResponds and fulfills internal and external medical record requests in accordance with organizational policies, compliance, and timeliness expectationsPrepares necessary data for the hospice interdisciplinary team (IDT) meetings in accordance with organizational policies, compliance, and timeliness expectationsSends appropriate patient updates to patients' provider organizational policies, compliance, and timeliness expectationsMaintains confidentiality and HIPAA compliance while working with protected health informationCollaborates with other departments to complete timely health information management duties
