Health Educator I - FT - Hackettstown - Atlantic Health System : Job Details

The Community Health Educator will implement the Community Health departments education and outreach plan, connecting and engaging with patients and community members through health and wellness events, collaborating with community partners and agencies, and direct outreach. This role is also responsible for assisting patients and community members to connect with community resources and referral appointments, overcome barriers to care and navigate the health care system, including community agencies and organizations. Lead the implementation of systemwide Community Health initiatives and oversee deployment of programs and community engagement activities within the assigned medical center region. Develop and deliver health education by researching, creating, and implementing evidence-based curriculum and materials to promote awareness and access to health services across the lifespan.
