Job Description PURPOSE OF POSITION: Under general supervision of the Assistant Director of Nursing and the Director of Nursing in Ambulatory Care, directs the development, implementation and evaluation of the nursing regimen for patients/clients within the pediatric clinic, involving the diagnosis and treatment of responses to actual or potential health problems by means of nursing methods and techniques such as case-finding, health teaching, health counseling and the provision of care supportive to, and restorative of, life and well-being. Schedules, assigns, guides, directs and instructs nursing personnel in nursing activities related to chronic disease management in the pediatric population. Collaborates with the leadership team to ensure performance improvement opportunities are identified and implemented within the clinic. DUTIES & RESPONSIBILITIES: PRIMARY: • The Head Nurse will systematically evaluate the quality and effectiveness of pediatric nursing practice and nursing services, analyze appropriate data and information to identify opportunities in collaboration with ambulatory care nursing and physician leadership for improving services and patient outcomes. • Incorporates evidence-based knowledge into the identification of best practices in the delivery of nursing care to the pediatric population. • Collaborates with the Head Nurse of the pediatric clinic to identify gaps in pediatric disease management. • Collaborates with the Director of Ambulatory Care nursing quality to strategize and prioritize areas of improvement related to chronic disease management and population health. • Supports the nursing quality department and pediatric clinic HN in creating, implementing, and evaluating performance improvement initiatives related to pediatric chronic diseases. • Serves as a subject matter expert related to diabetes management and will get certified within one year of employment as a certified diabetes educator (CDE) • Provides Diabetes mellitus education for patients and their families/guardians during clinic appointments. • Educates patients and their caregivers: CDEs teach patients and their families about diabetes and how to manage it. They also help patients identify and make lifestyle changes. • Manages programs: CDEs plan, implement, monitor, and evaluate programs to encourage healthy lifestyles. They may use data to identify community needs. • Links systems: CDEs may connect patients, health providers, insurers, and health systems to address health needs. • Stays current: CDEs maintain their knowledge of the latest information about diabetes. • Educates diabetes team: CDEs can keep other members of Heath Care team informed about new technologies, new medications and changes in the diabetes management guidelines. • Performs major duties and functions as described for the staff nurse, as needed. • Performs related managerial duties as assigned. OTHER: • Supervises, educates and orients professional and non-professional staff related to chronic disease management and population health. • Treat co-workers, patients, and families with dignity and respect. Is open and responsive to the diverse background and experience of others and fosters an environment that is sensitive to cultural diversity. • Serves as a patient advocate. • Ensures patient care is provided in accordance with established nursing standards of care. • Serves as a resource person and role model for staff. • Mentors, educates, motivates and fosters professional growth in staff. • Works collaboratively with Administration and Directors of Service to foster a team approach in the management of the practice. • Ensures processes/mechanisms are in place for proper follow-up of abnormal results and reporting Reportable Diseases/Conditions in a timely manner • Identifies and promptly addresses all patient safety issues including, but not limited to, hand washing, two-patient identification, etc. • Participates in performance improvement activities and projects and other committees as assigned. • Promotes open communication within the practice and conducts regularly scheduled meetings with staff. • Coordinates orientation for new staff and annual mandated in-services and other training for all staff. • Assist in the development and implementation of new initiatives, i.e. Patient-Centered Medical Home (PCMH). • Participates in facility-wide committees as assigned. • Assumes responsibility for professional growth. Maintains nursing skills through participation in continuing education programs. • Performs other duties as requested. SUPERVISES: 1. Assistant Head Nurses 2. Staff Nurses 3. Licensed Practical Nurses 4. Patient Care Associates 5. Public Health Advisors Minimum Qualifications 1. A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and 2. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA); and 3. One (1) year of experience as a registered nurse. Benefits In addition to base compensation, you may also receive the following based on the Collectively-Bargained Agreement: An annual education differential if you currently possess an appropriate degree in nursing or an allied health discipline from an accredited college or university, as listed below: Bachelors in allied health discipline = $900 Masters in allied health discipline = $1,200 Bachelors of Science in Nursing (BSN) = $1,800 Masters of Science in Nursing (MSN) = $2,000 Doctorate in Education, Nursing Practice or PhD in Nursing (DNP) = $2,500 An annual certification differential ranging from $1,878 up to $4,878 if you currently hold appropriate certifications that are nationally recognized in the discipline or area of specialization for which you are employed. An annual nursing experience differential ranging from $1,042 up to $8,645 for up to 10 years of experience if you have outside nursing experience as a Head Nurse.
