A health services network in Long Island is currently seeking a new Supervisor to join their growing Health and Recovery Plan (HARP) team in Long Island.ResponsibilitiesThe Supervisor will:
- Supervise a team of 6 care managers, each managing a caseload of 35 clients.
- Provide guidance, feedback, and support to care managers in high-risk crisis situations.
- Conduct fieldwork and client visits throughout Nassau County.
- Ensure care managers are trained and equipped to handle their responsibilities effectively.
- Maintain clear boundaries while being personable and approachable.
- Carry a small caseload of clients as needed.
- Collaborate with other supervisors and program directors to ensure seamless service delivery.
- Participate in meetings and trainings at various locations, including Queens and Copiague.
- Assist care managers with the development of effective clinical interventions and care plans.
- Ensure staff is kept informed of Health Home policies and procedures as well as departmental priorities.
- Monitor Health Home compliance and quality assurance.
- Provide crisis intervention to clients in distress.
- Complete enrollment and service date tracking reports monthly or as needed.
- Review and approve documentation as assigned.
- Consistently exhibit the values of teamwork, open book management, advocacy, and customer service.
- Participate in quality assurance and quality improvement activities.
- Conduct staff meetings, trainings, and supervision.
- Facilitate bi-weekly Clinical Case Review with the Care Managers.
- Perform other duties as requested.
Qualifications:
- Bachelor's degree in Social Work, Psychology, Human Services, or a related field (Master's preferred).
- Minimum of 3 years of experience in care coordination or OMH residential services for adults.
- Supervisory experience is required.
- Fluency in Spanish or Creole is highly desirable.
- Strong leadership and organizational skills.
- Ability to handle multiple tasks and work in a fast-paced environment.
- Reliable transportation for fieldwork and client visits.
- Two (2) years post-graduate experience working with individuals or families who have a history of mental illness, substance use, homelessness, or HIV/AIDS.
- Excellent problem-solving, decision-making, and judgment skills.
- Ability to work on an interdisciplinary team.
- Excellent interpersonal and time management skills.
- Preferred: MSW/LMSW/LCSW, 2 years working with severe mental illness population, 1 year supervisory experience.
