To provide clinical counseling services to patients and families of the Addiction Institute Halfway House. Is cognizant of the philosophy, standards, objectives and policies of the Department and the Hospital Center. Responsibilities A. CLINICAL/ TECHNICAL/ SERVICE - Demonstrates the ability to perform clinical/ technical/ service/ administrative tasks - Attends and participates in team meetings - Prepares intake psychosocial, initial treatment plan, recovery and discharge plans for patients on caseload - Does individual and group counseling - Documents treatment in medical record via EMR systems - Presents at case conferences - Manages the care assigned to patients on caseload - Participates in clinical supervision and utilizes direction - Provides reports of clinical services to supervisor when requested - Make referrals to appropriate resources for the ongoing care of patients - Provides support counselling, including at least bi-weekly appointment with each patient on caseload - In...Supervisor, Healthcare, Patient, Clinical, Treatment
