Your Role as a Guidance and Transition Coordinator
  • Facilitate Transition Classes & Seminars - Deliver group classes and workshops for 11th and 12th grade students focused on post-high school planning, including life skills such as budgeting. Provide engaging, age-appropriate content to support student preparedness for life after graduation.
  • Provide Individual & Small Group Student Support - Facilitate 1:1 and small group counseling sessions to address students' academic, social, and emotional needs. Offer regular check-in support for students requiring additional guidance.
  • Design & Implement Transition Curriculum - Develop and teach engaging lessons for middle and high school students that support transition planning and align with curriculum goals. Adapt instruction to meet diverse student needs and ensure curriculum relevance.
  • Lead Transition Planning Meetings - Conduct biannual transition planning meetings with 11th and 12th grade families to review post-secondary goals. Offer optional meetings for 9th and 10th grade families to encourage early engagement in transition planning.
  • Support Academic Progress & Record-Keeping - Assist with student course scheduling, monitor and track credits, and maintain accurate high school transcripts and GPAs to ensure students remain on track for graduation.
  • Guide College Application & Financial Aid Processes - Support students and families with the college application process by providing resources on financial aid, assisting with application steps, and coordinating teacher recommendations.
  • Plan & Implement Parent Workshops - Organize and facilitate transition workshops for parents, focusing on topics such as college and career planning. Equip families with the knowledge and tools needed to support their child's future.
  • Coordinate ACT & Practice Exam Administration - Oversee the implementation and administration of the ACT and related practice assessments, ensuring students are well-prepared for standardized testing requirements.
  • Develop & Monitor Individualized Transition Goals - Work collaboratively with students and families to set personalized transition goals. Guide parents in understanding and supporting these objectives to promote student success.
  • Collaborate with Faculty & Administration - Communicate regularly with faculty and administration via email and in meetings (including team and case study meetings) to support student transitions. Share insights and updates to ensure cohesive support across all stakeholders.
Qualifications:
  • 1-2 years experience in special education setting.
  • Bachelor's or Master's degree in Counseling, Social Work, Psychology, Special Education, or a related field from accredited college or university.
  • NY certification as a Guidance Counselor, School Counselor, School Social Worker, or School Psychologist.
  • Strong writing skills required for regular email communication and narrative report cards
Preferred/Nice to have:
  • Experience and comfort with classroom and behavior management
