Come Join our Team! Jewish Family & Community Services is a non-profit Social Services agency which provides various programs including guardianships, counseling and case management. Jewish Family & Community Services is a historic provider of social services and is a respected member of the Youngstown human services network. Jewish Family & Community Services is incorporated under the Youngstown Area Jewish Federation, which supports and monitors the services and budget. The Guardianship Manager will help to support the daily operations of the organization and in accordance with policy, guidelines, and procedures set forth by the Board of Directors, government regulations, and health insurance providers to assure the highest degree of care continues to be provided. All operations, decisions and planning shall reflect the philosophy of the Jewish Family & Community Services Mission Statement. DUTIES AND RESPONSIBILITIES:
  • Completion of all required documentation (guardian reports, etc.) in timely fashion to adhere to probate court timelines.
  • Ability to make critical decisions on behalf of wards and their care.
  • Assure that HIPPA and Confidentiality are maintained at all times.
  • Regular communication with wards, family members of wards and related team members (at nursing homes, residence, etc.) to ensure needs are being met.
  • Assessing each ward's living arrangements, on an ongoing basis, to ensure their individual needs are being met.
  • Applying for benefits on behalf of wards (Medicaid, Medicare, VA etc.) as needed.
  • Completion of bill payments, on behalf of wards, with office admin support.
  • Management of wards estate/finances as needed.
  • Regularly attend plan of care meetings for individuals placed in nursing home facilities.
  • Making all necessary referrals on behalf of the ward's best interest.
  • Coordinated and/or transport wards to appointments as needed.
  • Respond to all emergencies of wards, as required, after hours or on weekends.
  • Documenting case activities on a regular basis.
  • Annual Completion of all required Ohio Adult Guardianship Education Programs per Probate Court.
  • All other duties assigned.

Requirements Bachelor's Degree (minimum) in social work field or other related background; Prior experience with guardianship case management is desirable; Working Knowledge of public benefits (Medicaid, Medicare, etc.) required; Must be sensitive to needs of elderly, mental health and developmental disabilities population; Must be able to act independently, be detail oriented and possess a professional work ethic; Regular travel is required. Must have reliable transportation, valid driver's license and active vehicle insurance. Candidates must pass FBI/BCI background check and drug testing.
