Geriatrics Nurse Practitioner - Med Ninjas : Job Details

We are seeking a Locum Tenens Nurse Practitioner - Geriatric Medicine in Pennsylvania.

• Setting: PACE (Program of All-Inclusive Care for the Elderly)

• Responsibilities:

  • Provide primary medical management in collaboration with PACE Physicians
  • Perform assessments and develop care plans for participants
  • Evaluate and treat participant physical complaints
  • Provide education to participants and caregivers
  • Function as part of the Interdisciplinary Team (IDT)
  • Provide oversight of the PACE Clinic
  • Perform rounds in contracted nursing homes
  • Document participant conditions and update medical records

• On-Call/Weekend Requirements:

  • On-call every third week (Monday Friday, 5:00 PM to 8:30 AM; Saturday and Sunday all day)

• EMR System: Epic

• Active PA license Required
• ANCC/AANP Board Certified Required
• BLS Certification Required
• CDS Certification & DEA Registration Required
• 5-7 years of NP experience, preferably in long-term care, with at least 1 year working with the frail/elderly population Required
• Clean/Clear malpractice history Highly Preferred
