Job DetailsJob Location Tennessee - Nashville, TN Position Type Full Time Education Level 4 Year Degree Travel Percentage Up to 25% Job Shift Day Job Category Nonprofit - Social Services DescriptionGap Services Case Manger (Nashville) The Gap Services Case Manager provides appropriate case management and supportive gap services to eligible ORR-eligible populations, including Ukrainian Humanitarian Parolees and Cuban/Haitian Entrants through outreach, intake and assessment, referral, and follow up. The primary focus of the Preferred Communities-Gap Services program is to provide eligible clients with those gap services along with referrals to allowable and ongoing Office of Refugee Resettlement (ORR) and mainstream benefits including cash and medical assistance, employment (Match Grant), and social services. The Gap Services case manager will also provide individualized health education and advocacy for the clients' access to healthcare. The Gap services case manager will provide necessary referral services to help clients become more socially integrated and culturally acclimated in their new communities. The Gap services case manager will provide support to all clients for a maximum of 90 days unless a client is especially vulnerable. In the event of a an especially vulnerable client in need of services beyond the 90-day service period, (large family, women-at-risk, elderly clients, clients that identify as LGBTQ, clients with significant physical or mental disabilities, and other vulnerable populations) the Gap Services case manager will refer clients to Intensive Case Management (ICM) for continued appropriate services. - Annual Compensation $40,000-$45,000
Responsibilities:
- Assesses clients for eligibility using Preferred Communities-Gap Services guidelines
- Provides translation/interpretation services as needed to English
- Provides direct services to/for Inspiritus clients as assigned in a professional, courteous, and caring manner
- Conducts outreach, intake and assessment, referral, and follow-up to eligible clients
- Conducts two home visits within the 90-day service period, including an initial home visit and safety check
- Maintains communication with clients every 30 days and documents communication as required by programmatic guidelines
- Provides referral for a domestic medical screening, tuberculosis screening and any necessary vaccines required for parole applications
- Provides training and education related to serving clients to community partners
- Assesses clients for housing and other immediate emergency needs as necessary
- Provides clients with linkages to cash and medical assistance, employment, and social services
- Assists clients and sponsors with applications for Social Security, Employment Authorization Documents, driver's licenses, school enrollment, etc. as necessary
- Provides vulnerable clients with referrals to Intensive Case Management as necessary
- Accompanies clients to appointments as needed for services (e.g. health screening, social security appointment, DFCS benefit enrollment, ESL/school registration/WIC)
- Assists with and educates on the completion and submission of forms or applications for appropriate services
- Completes appropriate employment referrals and assists with Match Grant (MG) service enrollment in coordination with the employment team to ensure appropriate employment placement as needed
- Submits all documentation required to prove that services have been provided (this may include copying original documents)
- Maintains all records and client files in an accurate, complete, confidential manner as required
- Keeps supervisor apprised of difficult cases
- Ensures that all services needed by the client are scheduled, provided, and documented in the file according to grant requirements
- Develops individualized, holistic service plans based on client needs and gaps in sponsor services
- Networks and develops personal contacts with other social service providers, including both mainstream providers and those that serve parolees exclusively
- Uses contacts and networking to create and maintain a database of client services to supplement those that Inspiritus provides
- Will provide health education and orientation to the American healthcare system to clients
- Refers clients to other programs as needed such as Economic Empowerment& Intensive Case Management
- Maintains personal and professional boundaries in the performance of the job
- Forges mutually respectful partnerships with clients, co-workers, and employers
- Participates in Inspiritus functions, training, fundraising events, public relations activities and other Inspiritus activities and initiates such as performance and quality improvement (PQI)
- Participates in in-service training and conferences related to the programs
- Conforms with and abides by all regulations, policies, work procedures and instructions
Qualifications
- Bachelor's degree in a social service or similar field and minimum of two years of previous direct work experience in a culturally diverse environment, or an equivalent combination of education and experience
- Ability to organize multiple client-related activities
- Excellent oral and written communication skills in English
- Ability to balance the needs of the client and the employer
- Conviction about the capacity of people to grow and change
- Ability to set limits and maintain the helping role of the practitioner Excellent computer skills, accurate typing, and knowledge of Microsoft Word, Excel, and Outlook
- Must have the ability to communicate with all persons in a culturally competent manner
- Must be sensitive to the service populations cultural and socioeconomic characteristics.
- Must own a personal multi-passenger vehicle, hold a valid Georgia/Alabama Driver's license, have a good driving record, and have appropriate personal auto insurance
- Or an equivalent combination of education, experience, competencies, skills, and knowledge that ensures the successful completing of the essential job responsibilities.
