Become the Case Manager You've Always Wanted to Be

  • Work collaboratively with an interdisciplinary team, crafting treatment plans based on individual patient needs.
  • Oversee the creation of tentative discharge plans and contingency strategies in coordination with the team.
  • Facilitate and manage patient discharge experiences for optimal outcomes.
  • Monitor patient experiences, focusing on quality, timeliness, and service appropriateness.
  • Lead weekly team conferences and coordinate treatment modifications as necessary.
  • Maintain thorough and compliant documentation for case management addendums.
  • Stay informed on relevant regulations, policies, and departmental operations.
  • Analyze case management reports, including Key Care Indicators, to devise action plans.
  • Navigate commercial contracts and understand payor requirements and recertification needs.
  • Participate in Acute Care Transfer (ACT) meetings to identify trends for improvement.
  • Engage with patients and families within 24 hours of admission to establish strong communication.
  • Assess patient goals and document findings within 48 hours of admission.
  • Educate patients and families about rehabilitation services and the Case Manager's role.
  • Schedule and facilitate family conferences as needed for comprehensive support.
  • Assist patients with securing resources to prevent discharge delays.
  • Ensure compliance in orthotics and prosthetics ordering and payment processes.
  • Make timely referrals and maintain clear documentation to post-discharge providers.
  • Ensure accurate discharge and payor-related information is recorded.
  • Contribute to the utilization review process through data collection and trend review.
  • Participate in on-call case management scheduling as required.

Qualifications

  • License or Certification:
    • Must be qualified to independently conduct assessments within the discipline's scope (e.g., RN, SW, OT, PT, ST, Rehabilitation Counseling).
    • If required, candidates must possess an active license for their discipline.
    • Must be eligible for CCM® or ACM certification upon entry or within two years.
    • CCM® or ACM certification required, to be obtained within two years of placement in the Case Manager II position.

Minimum Qualifications:

  • Nursing candidates must have at least an Associate Degree in Nursing, with BSN preferred; acceptable qualifications vary by state.
  • Other licensed or certified healthcare professionals must have a minimum of a bachelor's degree, with a graduate degree preferred.
  • Two years of rehabilitation experience is preferred.
