The future of medical treatment has arrived, but it's not evenly distributed. Innovative medical technologies are entering the market every year, but it takes up to a decade for US health insurers to actually cover those treatments for patients (and even longer outside the US), blocking patients from the best possible medical care. We are a fast-growing, Stanford-founded early-stage AI SaaS startup (with venture capital funding and a long runway). We use LLMs and AI agents to fight for medical coverage for individual patients.

Job Responsibilities We give major strategic ownership to everyone at the company.
- Feature Development: Design, build, and iterate on end-to-end product features (frontend, backend, or both) using modern frameworks and cloud services.
- Execution & Accountability: Turn product goals into scoped milestones, and keep the team unblocked and on-time.
- Product collaboration: Work with founders, design, and other engineers to translate user needs into simple, scalable solutions.
- Improve our generative AI capabilities. Foundational model training and fine-tuning won't be necessary, but we do a lot of vertical-specific LLM orchestration.
- Team & culture: Raise the bar via code reviews, documentation, and mentoring; help interview future teammates and shape engineering norms.
Qualifications Required:
- 4+ years of software engineering experience (full-stack preferred)
- Strong Python and JavaScript/TypeScript skills
- Comfort in ambiguous, fast-paced environments; self-starter mindset
- Detail-oriented, with a high bar for quality and trust in healthcare tools
Ideal, but not required:
- Startup experience, especially early stage
- Experience with LLMs in real-world applications
- Familiarity with React, Node, SQL, Docker/Kubernetes
Tech stack We use TypeScript in our frontend and Python for pretty much everything else. We run in a Kubernetes cluster on AWS, and use various AWS services (S3, Bedrock, etc) where it makes sense. Compensation We offer salary that is competitive with other startups, and a very generous equity package (which you'll get at a lower valuation before we raise our next funding round!). - Haley, our CEO, holds an MBA from Stanford and spent over six years at Medtronic in engineering, product management, and business development roles. She and Alex met as classmates during their MBA program at Stanford.
- Alex, our COO, has experience as a product manager at Meta and Microsoft, and as a serial side-project/startup founder. He also holds an MBA from Stanford.
- Malcolm, our third cofounder, joined during his last year at Stanford (where we convinced him to start Paxos with us rather than going to medical school), and he had previously founded and led a healthcare nonprofit.
- Ben, our founding engineer, holds an M. Eng. from Cornell, previously built his own ML startup, and has many years of experience with building innovative software products. He's also a lawyer.
