Position Overview

We are seeking a dedicated and compassionate Field Registered Nurse to join a highly rated Home Care agency serving patients in Bronx and Westchester County, NY. This role involves managing all aspects of patient care in a home care setting, including assessments, care coordination, and documentation. The ideal candidate will have at least 1 year of hospital experience and be willing to travel within the designated areas.

Qualifications
  • Education: Associate/Bachelor's Degree in Nursing.
  • Licensure: Active New York State Registered Nurse license.
  • Experience: Minimum of 1 year of hospital experience.
  • Technical Skills: Proficiency in home care assessments, care planning, and documentation using Home Care Home Base EMR.
  • Soft Skills: Strong communication, organizational, and problem-solving skills.
  • Additional Requirement: Willingness to travel within Bronx and Westchester County, NY.
Key Responsibilities
  • Perform a home care assessment to determine the patient's eligibility for services.
  • In conjunction with the patient's family and physician, develop and implement the Plan of Care based upon a comprehensive physical, psychosocial, and environmental assessment.
  • Provide skilled nursing care as described in the 485-Plan of Treatment.
  • Orient and supervise home health aide personnel in accordance with regulatory requirements.
  • Evaluate the effectiveness of interventions in accordance with the plan of care.
  • Identify the need for evaluation by other disciplines.
  • Observe signs and symptoms and changes in patient's clinical, psychological and functional status.
  • Educate, counsel, supervise patients and caregiver relating to disease management and medical regime.
  • Document in HCHB Clinical Software all assessments, treatments, and services provided and patient response to the treatment plan.
  • Case manage respective caseload which includes ongoing communication with MD, case conferencing with supervisor, obtaining updates from interdisciplinary team, completion of interim orders, updating of patient profile, conducting recertification assessments, etc.
  • Contact physician to report, clarify, and/or obtain orders for medication changes/additions, precautions, treatment, changes in visit frequencies, additional services needed.
  • Document and complete all assessment visits within 48 hours of the assessment date.
  • At the start of each workday, review any incomplete/follow-up items upon logging in to HCHB.
  • Seek supervisory guidance when handling complex care patients.
  • Complete 60-day summary on recertification 485, locator 22 area.
  • Regularly participate in case conferencing with supervisor and other members of the interdisciplinary team.
  • Conduct discharge planning activities.
  • Attend all mandatory in-services.
  • Participate in Quality Improvement activities.
  • Observe principles of Infection Control and adheres to standard precautions at all times.
  • All other duties as assigned.
