Family Worker - GOOD SHEPHERD SERVICES : Job Details

The Family Counseling for School Success Program provides counseling and case management services to families connected with PS/ MS 15, PS 291, PS 246 or live within the community. The Family Worker will provide supportive case management services to assigned families utilizing strength-based engagement strategies. The Family Worker will work closely with the Case Worker/ Social Worker to ensure effective and efficient coordination and collaboration of services. Case Practice Responsibilities: Conducts a concrete needs assessment for all assigned families. Connect families to resources to address identified needs. Assist participants to successfully connect to resources, such as completing application and referral forms, helping with transportation and/or accompany to appointments; etc. Follow-up with providers on status of referrals and ongoing services.
